Think of Year 4 as your Revalidation Buffer Year.
Because revalidation occurs shortly after your 5th-year appraisal, this is your final opportunity to spot and fix any gaps in your supporting evidence before your Responsible Officer (RO) reviews your portfolio to make a recommendation to the GMC.
To help you get revalidation-ready, members of our Responsible Officer Panel here at MEDSU share how our ROs review your portfolio and what you need to focus on in this final year.
How a Responsible Officer Reviews Your Portfolio: The 12-Point Audit
When an RO opens your portfolio, they step back to paint a complete picture of assurance: Is this doctor up to date, fit to practice, and meeting all minimum requirements without concern?
Here is what they are checking behind the scenes, along with what you should do right now:
RO Checkpoint |
What the RO Looks For |
Your Final-Year Action |
|---|---|---|
| Appraisal Continuity | Yearly engagement or clear rationale for gap years. | Ensure all 4 previous appraisal summaries are uploaded. |
| Scope of Practice | Full coverage of all roles and locations every year. | Update your profile to include all current work settings. |
| PDP Alignment | PDP covers your whole scope and shows annual progress. | Draft a PDP that reflects every aspect of your clinical work. |
| CPD Requirements | Evidence that learning requirements are actively met. | Attach certificates and reflective logs for recent learning. |
| Complaints & Concerns | Full documentation, reflections, and learning outcomes. | Upload evidence of resolution and personal learning points. |
| Significant Events | Clear description of involvement and reflection. | Ensure all mandatory event declarations are complete. |
| Quality Improvement | QIA relevant to your specific operational role. | Complete at least one audit, clinical trial, or QIA loop. |
| Formal Feedback | Validated patient and colleague feedback loops. | Check that feedback meets minimum response thresholds. |
| Appraiser Summaries | Agreed outputs and documented appraiser notes. | Confirm all appraisal outputs are signed off by both parties. |
| RO Handover | Seamless transition data if you changed ROs mid-cycle. | Verify previous RO transfer details are on file. |
| Professional Standing | A current, signed Declaration of Professional Standing. | Complete your annual self-declaration forms. |
| Employer Assurance | No outstanding fitness-to-practise issues reported. | Confirm your employer has no pending governance checks. |
Demystifying Revalidation Deferral (And Why Feedback is Key)
The good news? ROs make positive recommendations to the GMC for roughly 85% of doctors.
If an RO cannot confirm that a minimum requirement has been met, they will defer your revalidation date. A deferral is a neutral administrative hold—it does not affect your license or ability to work. It simply grants you extra time to gather missing evidence.
The #1 reason for a revalidation deferral is incomplete feedback activity. Common feedback traps include:
- Starting too late: Running out of time to gather required responses (minimum 20 patients and 10 colleagues).
- Missing collation: Raw forms collected, but no summary report generated.
- Skipped reflections: Feedback collected, but not formally discussed and reflected on with an appraiser. (Note: MEDSU can arrange a dedicated 30-minute MSF session with your appraiser to fulfill this requirement).
- Invalid tools: Using informal questionnaires that don’t meet GMC standards.
- Expired data: Submitting feedback carried over from a previous 5-year cycle.
